• Membership transactions will take place at the front desk of the hospital
• Membership cards are issued upon receipt of payment
• Each month a new membership card of a different color will be issued
• Monthly memberships will not be pro-rated.

Orientation

• A “one-time” 30 minute orientation with a physical therapist, physical therapist assistant, occupational therapist, occupational therapist assistant, or wellness center attendant will be offered upon initiation of membership
• The goal of the orientation is to educate and instruct members on the functional process of the Wellness Center and to assist in establishing an individualized exercise program
• The orientation must be scheduled during the hours of 7:30AM and 6:00PM Monday - Friday
• To schedule an orientation call the Rehabilitation Services Department at 715-822-6167.
  

Past Medical History

All individuals are required to complete the “Medical History” document upon initiating their membership to the Wellness Center. This document will be provided by CMH-ECU at the time of registration.

• The “Medical History” document will be reviewed by the healthcare professional completing the scheduled orientation. If upon their assessment they determine there to be precautions or contraindications to exercise, physician clearance will be required prior to utilizing the Wellness Center.
  

Wellness Center Code of Conduct

• Membership card must be presented each time the facility is utilized
• Members must sign “in” and “out” on the provided log sheet each time the facility is utilized
• Individuals under the age of 15 are not allowed to utilize this facility
• Children may not be left unattended in the “Rehabilitation Services” waiting area
• Conversational volume must be kept at a manageable level out of consideration for those individuals receiving formal treatment
• Footwear must be dry and clean
• Members must provide their own towels
• All equipment must be wiped down following each activity using the provided cleaning solution and towel
• Active physical therapy, occupational therapy, or speech therapy patients always have priority over all equipment
• Treatment rooms/areas and plinths are not to be used by Wellness Center members.The Wellness Center is an independent exercise facility. Day-to-day progression/assistance will not be provided by the therapists
• The “Wellness Center Attendant” will be available to answer questions and assist during the evening and Saturday hours. During regular clinic hours please ask the Rehabilitation Aide or Rehabilitation Coordinator for assistance if needed
• Please know that CMH-ECU reserves the right to dismiss any individual from the Wellness Center if policy is violated.

Single membership: $30.00
Family membership: $40.00
Single senior membership: $20.00
Family senior membership: $30.00
Single CMH-ECU & Clinic Employee Membership: $20.00
Family CMH-ECU & Clinic Employee Membership: $30.00
**Senior memberships: 60 years of age and older.
**Family memberships: 2 adults and immediate children 15-21 years of age.
